Incident Repository Overview
The Incident Repository is a digital logbook where your safety team documents and tracks all safety-related events. It replaces manual spreadsheets and scattered files with a centralized, searchable database.
Purpose
The Incident Repository exists to:
- Preserve institutional knowledge — Incidents are recorded and retained, not lost to memory.
- Enable pattern recognition — Search past incidents to identify recurring issues.
- Support accountability — Document who, what, when, and follow-up actions.
- Facilitate LEO coordination — Provide records for law enforcement if needed.
What Gets Recorded
Typical incidents include:
- Medical emergencies
- Behavioral disturbances
- Trespasser encounters
- Criminal Trespass Warnings (CTW)
- Property or safety hazards
- Near-misses and observations
[!NOTE] Not every event requires a formal incident report. Use your organization’s guidelines to determine when documentation is needed.
Incident Data Model
Each incident entry captures:
| Field | Description |
|---|---|
| Date | When the incident occurred. |
| Reporting Member | Who filed the report. |
| Type | Category (Medical, Behavioral, CTW, Other). |
| Subject/Keyword | Short identifier or keywords. |
| Status | Open, Under Review, Closed. |
| Attachments | Written reports, photos, or related documents. |
Viewing the Repository
- Navigate to Incidents from the sidebar.
- You’ll see a list of recent incidents.
- Use filters to narrow by date, type, or status.
- Click an incident to view details and history.
[!IMPORTANT] Access to the Incident Repository may be restricted based on your role. Not all members can view all incidents.
